  1. Stage II (Dukes' B2 or B3) 
  2. TNM T3-4, N0, M0
  3. The cancer has spread through the muscular wall of the intestine or has extended to adjacent organs through the structures, but has not spread to lymph nodes . Dukes' B2 extends through the bowel wall; B3 involves adjacent structures.

  5. Standard Treatment  Surgical removal of the primary tumor , with or without adjuvant chemotherapy . Specific adjuvant therapy cannot be recommended for intermediate-risk tumors outside a clinical trial. But since a significant percentage of Stage II
  6. patients will develop recurrences,
  7. they should be considered
  8. candidates for clinical
  9. trials of adjuvant
  10. therapy.
